The Personnel Evaluation Form is a standard form used by the Star Army of Yamatai to evaluate the performance of Star Army personnel.

This form was most recently updated in YE 46.3.05.

Be sure to note whether or not the completed evaluation form will be made available to the personnel being evaluated. A Yes means they will receive a copy of the evaluation. A No means that they will not receive a copy of the evaluation.

In the Purpose of Evaluation field, indicate the reason for the evaluation. Is it an annual or otherwise periodic review, a correctional action review, a review intended to evaluate the subject for early promotion or some other duty?

An effective evaluation reviews the subject’s performance, and notes key points in their execution of their duties. Consider evaluating a subject in the following areas:

• In what ways do they live by the Star Army Creed?
• Evaluate their technical and tactical proficiency.
• How effectively do they communicate?
• Describe how they exhibited effort, as well as personal discipline.
• In what ways did they contribute to the team?

The optional plan of action section allows the evaluator to consider and recommend actions involving the subject. If this form is meant to be seen by the subject, it could include specific, measurable and realistic objectives that the subject should endeavor to achieve. If the form is not meant to be seen by the subject, it could include recommendations for actions command might take to help the subject improve in their performance.
